We need a permission slip for each event that takes place off campus. Thanks for participating!
The Diocese released a COVID-19 waiver for all volunteers and youth participants. Volunteers may sign one waiver for the entire year. Teens must sign a waiver to participate in all activities on campus, and then a waiver for each off-campus activity. See below for the links:
